The FLEAP Program Descriptions and Requirements handbook is a must read! It contains basic information, definitions, FAQs, directions for completing the forms below and much more! Faculty can access and download the handbook via Sentry Secured Services. The handbook is located in the "Faculty" section after login.
The Intention Form is the first step in identifying faculty who are interested in creating or resubmitting an existing FLEAP.
The Approval Form is used to record approvals as the FLEAP Proposal Form and/or Site Assessment Form moves from one administrator to another. It provides a status update and feedback as your FLEAP moves through the process.
The Proposal Form lists all necessary information that needs to be provided for the approval of a FLEAP. (An Approval Form MUST be submitted with the Proposal Form)
The Site Assessment Form is utilized when a site visit is proposed in order to investigate arrangements for lodging, classroom space, excursions, travel time between locations, transportation modes, etc. for a FLEAP. (An Approval Form MUST be submitted with the Site Assessment Form)
