This course will focus on key initiatives to improve healthcare outcomes and lower costs. Students will develop a heightened understanding of the range of personal social economic and environmental factors that influence healthcare outcomes. Students will learn to manage the various aspects of transitional care through a focus on building relationships interpersonal communication and team collaboration in the care coordination process. Practical and strategic approaches that promote positive outcomes for patients and families will be examined.3 Credits
